- The intended outcome of ACP is a statement of wishes, although the ACP outcome is not legally-binding
- The ACP outcome may lead to legally-binding documents such as an AMD or an LPA
- Individuals should be encouraged to involve family in the process so that their wishes may be understood
- The patient can terminate an ACP discussion at any time
